HOW TO ENROLL
Please fill out the form to the right to apply to participate in the San Jose COVID Food Relief Program and be eligible to receive free groceries or prepared meals.
WHO QUALIFIES
Residents in San Jose who need access to healthy and nutritious meals and meet one of the following criteria:
– Any individual that meets the CDC definition of individuals at high risk for severe illness from COVID-19
– Individuals who have suffered from employment or business interruptions due to COVID-19 related business closures and do not currently have other assistance in purchasing food or cooking meals on a consistent basis.
DISQUALIFIED
Anyone who currently receives state or federal food assistance such as CalFresh or other home-delivered meals or groceries.
